- BIR Certificate of Registration (Form 2303)
- Mayor's/Business Permit
- Articles of Incorporation and By-laws (for corporations)
- Valid government-issued ID of the company's authorized representative
- Sketch or location map of the business address
- List of initial employees with start dates and salaries
Agency-Specific Notes:
SSS (Social Security System):
Employers must accomplish the SSS Form R-1 and R-1A for employer and employee enrollment and submit monthly contributions once employees are onboarded.
PhilHealth:
Requires submission of Employer Data Record (ER1) and Report of Employee Members (ER2). Contributions are based on a tiered income bracket and submitted monthly.
Pag-IBIG (HDMF):
Employers file the Employer's Data Form (EDF) and Membership Savings Remittance Forms. Pag-IBIG contributions are also monthly and partly shouldered by the employer.
